Medicare Lifestyle Coaching Package includes:
- Two 50-minute sessions with a Lifestyle coach
- Ethos Dietary Guide
- Care Plans
When you meet Dr. Weiss for your fist visit, it is a time for you to be heard. In this introduction session, Dr. Weiss gets to know you, answers your questions, and inquiries about your goals towards increased health.
The second visit is a lab review. Dr. Weiss will go over your lab results in depth and talk to you about how these indicators reflect the probability of such factors as Heart Disease, Cognitive Decline, Diabetes, Cancer, and more. He will speak to you about how you can improve in different areas with Lifestyle Medicine.
Coaching is an evidence-based practice that develops accountability and has been shown to improve the positive outcomes of working with a Lifestyle Physician.
The first Coaching session (3-7 days after Dr. Weiss's first visit) will expand on the understanding you gained from Dr. Weiss's visit. The coach conducts a lifestyle evaluation focusing on your choices in food, physical activity, sleep habits, mindfulness practice, harmful substances, social connections, and connection to the natural world in a non-judging way, enhancing your knowledge. There may be a possibility for you to take simple action steps immediately to improve your well-being.
The second Coaching session (3-7 days after Dr. Weiss's second visit) focuses on creating a long-term action plan to be on a preventative path and/or reverse the disease and start living an active, vibrant life with less stress and chronic disease.
BONUS: You will have access to the Ethos' Lifestyle coach for questions and concerns about your wellness goals that may come up between the two doctors' visits.
This option is for Medicare patients only. Medical visits will be billed to Medicare. You will be required to pay your copay, coinsurance, or deductible (if any) at the time of service. The cost of the New Patient Special does not cover any deductibles required by your supplement or Advantage plan.
* Dr. Weiss's medical visits will be billed for all in-network with eligible Medicare plans.
