Refund policy

Returns
Under no circumstance shall any fee be refunded or returned. All sales are final. All sales are nontransferable. Finals purchase includes the acceptance of the below agreement.

Dear Participant,
Thank you for your interest in Ethos Primary Care’s Nutrition/Lifestyle Education Class/es, visits, and programs. This is not a medical plan, an insurance program, or a supplement for medical care or coverage. This Participation Agreement describes Program participation benefits, as well as participant obligations. With final purchase, you accept the terms of this Participation Agreement.

Nutrition/Lifestyle Education Class/es, visits, and Program Benefits
As a Program participant, you shall receive the following:
• Nutritional/lifestyle education either online or in-person depending on your purchase.

You shall be provided advance notice of the Program's available group sessions by email to the address provided by you. Provision of a valid email address, receipt notification of group sessions, and the requirement to register in a timely manner shall at all times remain your responsibility.

Nutrition/Lifestyle Education Class/es, visits, and Program Goals
While each participant has their own personal objectives and reasons for participating in the Program, Ethos Primary Care is committed to furthering the following goals:
• Becoming self-sufficient and confident in living a plant-based whole foods lifestyle;
• Prevention and combating of chronic illness.
• Prevention and combating of the diseases of aging.

Please be advised that the nutritional/lifestyle educational and support components of the class/es are not covered by insurance and 100% of participation costs are your responsibility.

Under no circumstance shall any fee be refunded or returned.
No refunds will be provided due to technical issues on the participant's end. We cannot resolve any technical issues or difficulties that participant face when using their equipment to join the class.

You also accept the terms that the sessions are not private and hence when you share or ask questions you will be sharing with all who participate. Our sessions are recorded and there are no benefits offered if the recording is used for Ethos Primary Care use.

Other than the aforementioned, the following out of pocket expenses will be incurred by the Nutrition/Lifestyle Education Class/es, visits, and Program participants:
• Cost of personal groceries
• Cost incurred by a participant to travel, attend or participate in any Program consult, educational course, training session, or meeting
• Cost of a significant other/family member that you bring to a class
• Any additional expense incurred by you as a result of your participation excluding those services including in the Program Benefits section above. l

Guarantees
There have been and can be no representations or guarantees made by Ethos Primary Care regarding the outcome of your Program participation. Your participation is optional, and by agreeing to participate you understand and acknowledge the benefits you may receive from participation will likely directly correlate to your participation in offered education and training programs, as well as your commitment and adherence to suggested Program lifestyle adaptations.

Nutrition/Lifestyle Education Class/es, visits, and Program Participant Expectations
As a Program participant, you are expected to utilize your best efforts to adhere to the Program and to (1) prepare for all Program sessions in advance as specified; (2) attend all scheduled sessions purchased by you; (3) complete any and all assignments and adhere to your personal instructions provided by Ethos Primary Care; (4) participate in Program offered educational and training sessions; (5) take responsibility for tracking Program progress; and (6) treat other Program participants with respect at any and all group sessions.

Failure to Perform
Ethos Primary Care’s failure to conduct any Program educational, training group, or personal session for any reason shall not be construed as a breach of this Participation Agreement. Ethos Primary Care will utilize its best efforts to notify you of any cancellation and/or rescheduling of any training or educational Program activity in advance.

The Program is not an insurance plan and is not intended to replace or supplement your health insurance coverage.
If at any time you are in need of immediate medical assistance, it is recommended that you dial 911.

Indemnification
You hereby agree to indemnify and hold harmless Ethos Primary Care, P.C. (and its owners, shareholders, directors, employees, agents, and representatives, including Dr. Ron Weiss) against any cause of action, claim. damage, liability, judgment, verdict, cost, fee, or expense (including, without limitation, reasonable attorneys’ fees and other consultation fees and court costs) arising in any manner from or with respect to your participation in the Program.

Governing Law/Venue
This Agreement shall be governed by and construed in accordance with the laws of the State of New Jersey without giving effect to the rules of conflicts of law and the venue for any action shall be the State of New Jersey.
